Instructions to Artists:


Artist Statement         

 

  • Artist statements are important; they explain how your creative process works and who you are as an artist. The artist statement should be approximately three paragraphs which fit on one 8 x 11 ½ page. If you wish you can include picture(s) of your artwork.
  • Send your statement via email to Betty, bettyhulse@yahoo.com, or if you do not have email, send to her at 940 Grandview Ave, Reno, NV 89503.
  • Please keep a copy for your records. 

Artwork Entry Inventory Form

2010 Interwoven Inventory Form 

 

  • A signed printed copy of this form must accompany your artwork.  This is your loan agreement to the Nevada State Museum.
  • This Inventory Form can be filled out on your computer.  To do so, you should open it, then immediately “Save As” a new document name, so that the original form is not changed (that way it can be re-used if needed).
  • You will number your submissions on the inventory form.  Entry No. consists of artist’s three initials + 1, 2, 3, 4, or 5.  Example: Betty Hulse’s first basket listed would be BKH1.
  • You can print out your typed inventory, which MUST accompany your basket(s) submissions. You will also need to email a copy to Betty Hulse, bettyhulse@yahoo.com.
  • For those without computer access, this packet contains the forms to be filled out. Please print clearly and send a copy to Betty Hulse, 940 Grandview Ave, Reno, NV 89503.
  • Please leave the date in/out column blank. Please keep a copy for your records.

  • A printed form for each entry MUST accompany your artwork.  It is important that the information about the artwork is accurate because it will be used for the labels and the catalog.  After your artwork is photographed at the Museum, a picture of your artwork will appear in the upper right-hand corner where a picture now appears.
  • Use the same Entry Number for each piece that you used on the Inventory - your three initials and 1, 2, etc.  This number will become your tracking number. I
  • If you have computer capability, email a copy of each entry to bettyhulse@yahoo.com.
  • For those without computer access, please send a copy to Betty Hulse, 940 Grandview Ave, Reno, NV 89503.
  • Please keep a copy for your records.

 

Vendor Forms and Consignment Forms

 

  1. For those artists who will be selling their artwork at the Museum, State of Nevada Vendor forms and Museum Consignment forms can be obtained here: State of Nevada Vendor Form and NSM Consignment FormIf you are selling your artwork you will need to send the museum an invoice upon receiving a purchase order.  Please print a copy of this form Invoice Form.
